How do I dematerialise the shares that I hold?
Visit your nearest Axis Bank Branch for conversion of your Original Physical Share Certificates into dematerialised form.
Dematerialisation Request Form (DRF) duly filled and signed by all the Demat account holders along with the original physical share certificates would be required to process this request.
Please note that Share certificate must be on the name/s in whose name/s Demat account is opened. 
If the securities for Demat are available with NSDL and the request is found in order, the same will be processed at our end and will be forwarded to the concerned Registrar and Transfer Agent (RTA) of the company for further processing and crediting the shares in your Demat account. 

What should I do to dematerialise shares and the ownership of shares is not in order?
If the ownership of shares is not in order a transposition form has to be submitted along with the DRF while submitting the share certificates for Dematerialisation. 
i.e if the Shares are in the name of A & B and account is in the name of B & A , shares can be dematerialised in the said Demat account if transposition form is submitted. However the said is not true in case of Mutual Funds 
Also, if the Demat account is in the name of A, share certificates in the name of A and B or B only, then the share Certificates cannot be dematerialised in the said Demat account. You will then have to open a new Demat account as per the order of the share certificate.

How do I dematerialise the Mutual Funds I hold?
Visit your nearest Axis Bank Branch for conversion of your Mutual Fund (approved) units into dematerialised form. 
A Conversion Request Form (CRF) duly filled and signed by the Account Holder/s along with the statement of accounts showing proof of the holding of mutual funds will be required to process this request.
